Consent to Delete Data Form


When devices are submitted to a forensic lab and are ready to be returned or released, they may contain data that should be destroyed and not released outside of the forensic lab. In these circumstances, this document can be used to obtain consent to forensic delete certain files or sanitize an entire device, making that data irrecoverable.


This customizable form in Word format allows forensic labs to receive written consent from a device's owner or custodian to permanently delete data. Use cases include digital evidence that includes contraband, proprietary information, or other data that should not be released back into the possession of the device owner. This may include permanently removing specific files (i.e., deleting files and then overwriting unallocated space) or forensically sterilizing an entire device.
